3. Does your MDT have an operational policy in place, which is reviewed annually and distributed to all members of the team?

This policy, which should be agreed by the whole team, should include the following:

• A list of team members.
• Role of the team members.
• Named lead clinician.
• Responsibilities of the lead clinician.
• Organisation of the Meeting.
• Role of the MDT meeting co-ordinator.
• Policy for reviewing patients in the meeting.
• Policy on communication with GPs and other key members of the team.
• Guidelines for referral from Primary care.
• Guidelines for referral to Tertiary care.
• TSSG agreed clinical guidelines.
• General instruction e.g. cover for Bank Holidays, and absence of members of the team.
